ResponsibilitiesThe Clinical Pharmacist 1 provides foundational pharmacy services, including medication order verification, dispensing oversight, drug information support, and basic patient-oriented clinical pharmacy services. Reviews the accuracy and appropriateness of medications prepared and dispensed by pharmacy personnel and serves as a drug information resource to physicians, nursing staff, and other hospital personnel. Supports safe, accurate, and efficient medication use throughout the hospital. Job DescriptionEssential AccountabilitiesReflects the job's main responsibilities and is not intended to be an exhaustive list of all duties performed; therefore, its content does not restrict management's right to assign or reassign duties and responsibilities to individuals in this job.o Verify the accuracy and appropriateness of routine medication orders and prescriptions, including allergies, interactions, compatibility, dosing, and other safety considerations.o Verify the accuracy of medications compounded and/or dispensed from pharmacy services, including carts, trays, kits, and other medication distribution systems.o Accurately verify sterile and non-sterile compounds, as applicable to assigned work area.o Provide drug information and basic consultative support to physicians, nurses, and other disciplines.o Perform patient-oriented clinical pharmacy services, including medication-related assessments, dose calculations, and recommendations for therapy optimization within established guidelines and protocols.o Perform protocol-based interventions, such as IV-to-PO conversions, as applicable.o Direct pharmacy technicians and support staff in completing work according to current priorities and workflow demands.o Document pharmacist interventions and report medication errors and near misses in accordance with department standards.o Serve as a resource to new employees, pharmacy students, and residents as assigned.o Perform other job-related duties as assigned. Knowledge, Skills, and Abilitieso Knowledge of general hospital pharmacy practice and basic clinical pharmacy practice.o Knowledge of medication safety principles and pharmacy operations.o Ability to accurately interpret and evaluate medication orders.o Ability to communicate effectively with physicians, nurses, patients, and pharmacy staff.o Ability to prioritize work and maintain accuracy in a fast-paced environment. QualificationsEducationo Doctor of Pharmacy (PharmD) required Experienceo Hospital pharmacy experience preferred Licenses and Certificationso RPH - Registered Pharmacist with valid State of California Pharmacist License required Disclaimers
