Qualifications

• Current RN license and CPR certification required.
• Experience in home health care or healthcare management is essential.
• Knowledge of regulations, accreditation standards, and EMR systems necessary.
• Strong assessment, communication, and interpersonal skills needed.
• Proficiency in Microsoft Office Suite and analytical problem-solving required.
• Flexibility to work occasional night and weekend hours important.
• Must pass a criminal background check and drug screen.

Responsibilities

• Ensure accurate documentation and compliance with QA reviews.
• Lead patient recertification, hospitalization, and discharge processes.
• Oversee disease progression management, DME, and conduct retention visits.
• Supervise Case Managers, RN/LPN contractors, and intake nurses; assist with hiring and evaluations.
• Manage caseload distribution and monitor patient census for productivity.
• Maintain QA compliance and lead QAPI meetings to prepare for surveys.
• Ensure HIPAA compliance and adherence to all regulations and standards.
