Clinical Manager - Home Health

This is an excellent opportunity for an experienced Registered Nurse who enjoys combining clinical expertise with team leadership. As our Clinical Manager, you will help coordinate the delivery of high-quality home health services, support and develop our clinicians, and ensure patients receive timely, compassionate, and well-coordinated care.

You'll work closely with agency leadership and our interdisciplinary clinical team while playing an important role in the continued growth of Choice Home Health throughout the Monterey area.

What You'll Do

As the Clinical Manager, you will provide day-to-day clinical leadership and oversight for an assigned team of patients and clinicians. Key responsibilities include:
  • Provide clinical supervision, guidance, and support to members of the interdisciplinary care team.
  • Coordinate clinical services to ensure patients receive appropriate, timely, and high-quality care.
  • Assist in maintaining compliance with applicable federal, state, local, Medicare, and Medi-Cal requirements.
  • Review new referrals and help ensure assessment and start-of-care visits are completed timely.
  • Make effective patient care assignments based on patient needs, clinician competency, geography, and available resources.
  • Coordinate clinician schedules, staffing needs, and on-call coverage to support continuity of care.
  • Conduct and participate in care conferences, team meetings, and clinical in-service programs.
  • Monitor patient care plans and collaborate with clinicians to address changes in condition, barriers to care, and other clinical concerns.
  • Provide ongoing clinical guidance, coaching, and education to field clinicians.
  • Maintain appropriate records related to case assignments, staffing, and clinical management activities.
  • Serve as a clinical resource and help troubleshoot complex patient care situations.
  • Support clinical documentation standards and timely completion of required records.
  • Serve as a preceptor and mentor to clinical staff when needed.
  • Collaborate with agency leadership to identify opportunities to strengthen clinical operations, patient outcomes, and the overall patient experience.
  • Support the continued growth of Choice Home Health while maintaining a strong focus on quality, compliance, and patient-centered care.

What We're Looking For
  • Graduate of an accredited school of nursing.
  • Current, unrestricted Registered Nurse (RN) license in California.
  • At least two years of clinical nursing experience within the last five years.
  • Home health experience strongly preferred.
  • Previous supervisory, clinical leadership, or case management experience in home health preferred.
  • Working knowledge of home health clinical operations, care coordination, and documentation requirements.
  • Strong clinical judgment and ability to provide guidance to field clinicians.
  • Excellent interpersonal, verbal, and written communication skills.
  • Ability to recognize and respond appropriately to the needs and concerns of patients, families, and team members.
  • Strong organizational, prioritization, and problem-solving skills.
  • Ability to develop positive and productive working relationships across an interdisciplinary team.
  • Ability to make home visits and provide skilled nursing assessments and care when necessary.
  • Valid driver's license, reliable transportation, and automobile insurance.
