Position Summary:We are seeking a Clinical Manager to oversee patient care and supervise clinical staff. You will ensure care is coordinated effectively, services are delivered appropriately, and our team is supported in providing excellent patient care.
Key Responsibilities:- Review new case referrals and patient information to determine care needs.
- Assign appropriate personnel and coordinate admissions with the Medical Director.
- Supervise and guide clinicians to promote effective performance and high-quality care.
- Evaluate patient medical history, diagnosis, medications, procedures, and progress.
- Assist clinicians in setting therapeutic goals, care priorities, and care plans.
- Participate in case conferences to coordinate interdisciplinary care.
- Conduct quarterly record reviews and provide feedback to leadership and staff.
- Support hiring, onboarding, and orientation of new clinical staff.
- Help plan and implement staff training, continuing education, and organizational policies.
- Assess community health needs and assist in developing programs to meet them.
- Promote the organization's role in the community and foster a customer-service-oriented culture.
- Participate in quality assessment and performance improvement programs.
- Complete monthly billing workflow and other duties as delegated by leadership.
- Ensure compliance with professional standards and principles.
Qualifications:- Registered Nurse (RN) with current state licensure.
- BSN degree preferred, or minimum of one year as a professional nurse within the last three years.
- Previous hospice/home care experience and at least two years of supervisory or management experience.
- Strong understanding of hospice philosophy, pain and symptom management, and end-of-life issues.
- Excellent observation, nursing judgment, and communication skills.
- Proven ability to work effectively in an interdisciplinary setting.
- Licensed driver with insured, reliable vehicle.
- Current CPR certification.

Pay Range for Posted Region
$100,000-$120,000 USD
