St. Joseph's Center is looking for a Clinical Care Coordinator to lead nursing services within our ICF Programs. The Clinical Care Coordinator plays a key role in ensuring residents receive person-centered, high-quality care while guiding and developing our nursing and clinical staff. This position provides on-site clinical oversight at St. Joseph's Center locations. Responsibilities include supervising nursing personnel, conducting resident evaluations, ordering and managing medications, ensuring regulatory compliance, and supporting quality improvement initiatives across the interdisciplinary team.
What You'll Do- Provide oversight, supervision, and education to nursing staff, including evaluating clinical competencies and delivering training on disease management, infection prevention, medication safety, and wound care.
- Conduct routine medical evaluations, provide emergency care as needed, and manage medication orders and administration within scope of license.
- Ensure adherence to CMS and Pennsylvania ICF/ID regulations, audit nursing documentation, conduct mock surveys and environmental rounds, and support survey readiness.
- Participate in organizational committees and clinical policy development, working closely with the interdisciplinary team to support person-centered care.
- Assist with recruitment and retention efforts, supervise student nurse experiences, and support implementation of Electronic Health Records (EHR) and eMAR systems.

What's Required of You- Must have a degree for an accredited program recognized by the State Board of Medicine.
- Must have a valid Pennsylvania license as a Physician's Assistant or Nurse Practitioner
- Minimum 2 years of supervisory experience
- Must have 3-5 years of clinical experience
- Experience in long-term care, ICF/ID, or working with individuals with Intellectual and Developmental Disabilities, strongly preferred
- Required ID/D Credential within 2 years (must earn and maintain)
- Experience with regulatory compliance and survey process, highly desired.
- CPR Trainer, Medication Administration Trainer, Certified Investigator Certification, Preferred
- Valid PA Driver's License
