Job Summary

The Fetal Diagnosis and Treatment Center (FDTC) is a multi-disciplinary program that cares for mothers and their babies with congenital abnormalities. The FDTC is currently seeking an experienced nurse as a Clinical Care Coordinator, who will be responsible for coordinating all aspects of the patient's care from the initial referral through the patient care experience at Michigan Medicine. The Clinical Care Coordinator will promote open and collaborative communication among the multidisciplinary team, including sonographers, clerical staff, physicians, APRNs, and the patient and support persons. They will assess and address medical and psychosocial needs of patients. They will participate in the tripartite mission of the FDTC which includes clinical care, education, and research.

Responsibilities*
    • Attend conferences including fetal conferences and PAS conferences, and other meetings as necessary.
    • ?Reviews patient records and prepares information for PAS and MFM team use.
    • Performs patient intake and follow-up assessments in advance of patient clinical appointments in FDTC.
    • ?Maintains complete and accurate patient records.?Facilitates the development and execution of comprehensive and individualized management plan.
    • ?Oversees care coordination for FDTC complex fetal patients, assessing individual needs and assisting with navigation of the patient through planned care.
    • Maintains a comprehensive clinical patient list.
    • ?Advocates for individual patient needs by incorporating patient values in care planning.
    • ?Utilizes resources across the health system to coordinate clinical care?
    • ?Respond to patient treatment-related phone calls and inbasket messages.
    • Utilize MiChart functionality for communicating with patients and providers, for example, the portal, etc.
    • ?Assists physicians with clinical activities and procedures as needed.
    • ?Assists with the development of educational materials for specific disease diagnoses and program descriptions for patients
    • ?Provides education for parents/families; ensures communication of results and recommendations with referring providers?Provides phone and written follow-up for patients returning to their referring physician for care?
    • ?Collaborates effectively with a multi-disciplinary team, and outside providers.
    • ?Utilizes strong critical thinking and clinical judgment to troubleshoot and problem-solve system barriers to clinical care
    • ?Participates in collaborative research projects with multi-departmental staff
    • ?Participates in data collection including any QA/QI endeavors for the FDTC
    • ?Supports the advancement of the FDTC enterprise through participation in education, research, outreach, and marketing?
    • Other nursing duties as assigned. This may include supporting the other FDTC CCC teams.

Required Qualifications*

Required Educational Requirement; applicant must meet one of the following:

Bachelors degree in Nursing OR

Associates degree or diploma in Nursing and a Masters degree or higher in Nursing

Current license to practice as a Registered Nurse in the State of Michigan.

  • Minimum of 5 (five) years within the last 7 (seven) years practice as an RN in inpatient or outpatient OB/high risk. Outpatient preferred.
  • ?Participation in unit activities that demonstrate an ability to work collaboratively and positively contribute within a multidisciplinary team.
  • Excellent ability to prioritize work.
  • Experience with patient and staff education.
  • Experience with coordination of care. For example: coordinating outpatient schedules, telephone triage, procedure preparation.
  • Highly motivated with demonstrated ability to function independently and as a team member
  • Must demonstrate expert presentation skills, communication, organization, and prioritization skills
  • ?Demonstrated accountability/dependability as evidenced, for example, by a stellar attendance record; by completion of mandatories and unit-based competencies within an established time frame; by meeting deadlines and completing required work?
  • ?Demonstrated agility in managing rapidly changing situations effectively
  • ?Must possess excellent customer service skills?
  • ?Competent computer skills including EHR, Microsoft applications (word, excel).
  • ?Inpatient or outpatient certification in obstetrical care. Or successful completion within 1 year of hire date.

Hourly range for RSAM Registered Nurses $41.44 - $64.45 / hour

Evening Shift Differential-$3.00 / hour

Night Shift Differential- $4.00 / hour

Day Shift Weekend Differential- $2.90 / hour

Evening Shift Weekend Differential- $5.90 / hour

Night Shift Weekend Differential- $6.90 / hour

Charge Nurse Differential- $1.00 / hour

CCC- COMPETENT.

Actual RSAM LEVEL and salary will be determined at time of hire.

RSAM levels range from CCC-COMPETENT to CCC-EXPERT TO CCC-MASTERY to CCC- MASTERY PLUS.

Hours: Generally 8am- 4:30pm M-F 40 hours/week option with variable/flexible scheduling to accommodate unit needs
