Job Summary

This position will provide nursing support to patients transitioning out of inpatient rehab as well as outpatient needs within the division of Physical Medicine and Rehab. Care to include patients of Chelsea, Burlington and any ambulatory location within Michigan Medicine where Physical Medicine & Rehab is provided.

Responsibilities*
  • Candidate must perform duties independently, be highly motivated and flexible to address the needs of patients and their families.
  • Candidate will need to be familiar with internal and external resources for clinical assessment, decision making, and care management.
  • Provide nursing care to adult and limited pediatric patients and families in the Physical Medicine and Rehabilitation services (PMR)
  • Provide nursing care in collaboration with Physicians, other disciplines, and department colleagues for patients in clinic and via telephone to support efficient and effective management of patients.
  • Participate in the coordination of care, including testing, medication management, collaborative planning of care, support of self-management by patients/families, management of emergency situations, etc.
  • Provide patient and family education e.g., self-injection, medication management, preoperative and postoperative care, etc.
  • Function as a Patient Liaison for the PMR services to help coordinate visits for patients, help determine the service to best meet the patient's needs, and triage for same day/next day urgent visits.
  • Provide accurate and timely documentation of nursing care using an electronic medical record (MiChart).
  • Participate in quality improvement and LEAN thinking to improve workflow and processes.
  • May provide procedure support in collaboration with clinic support staff.
  • Provide nursing care for this patient population wherever PMR services are provided.

Required Qualifications*
  • Current license to practice as a registered nurse in the State of Michigan.
  • Bachelors degree in Nursing OR Associates degree/diploma in Nursing along with a Master's degree in Nursing
  • Two (2) or more years of nursing experience (in the last 5 years) in areas relevant to the care of musculoskeletal patients.
  • Minimum of 1 year experience specific to Physical Medicine and Rehabilitation patients

Actual RSAM LEVEL and salary will be determined at time of hire.

Shift/ Days/ Hours: Day shift; Monday through Friday; specific hours to be determined upon hire
Location: Chelsea Health Center/Burlington Building with possibility of providing care for patients at any Michigan Medicine locations where CMC/PMR services are provided

Note: All new employees will be expected to float to a designated unit(s) in times of low census.
