Job DescriptionWorking closely with the inpatient and outpatient child and adolescent interdisciplinary teams the Child and Youth Care (CYC) worker will collaborate on treatment and academic plans for patients in the REACH program, the community, conduct home, school and community visits to address and support school transitions, compliance, or lack of follow through with treatment and provide family with support and coaching while connecting them to appropriate levels of services.
The CYW will work closely with the patient, families and the team to address school related issues and transitions and ensure that they have adequate skills for daily activities and transitions back to home schools. The worker will accompany patients to school if school compliance is an issue and will work closely with the REACH program, HDSB, Mental Health Liaison Nurse/ clinical staff to ensure the patient is receiving appropriate school accommodations.
From the inpatient unit, the CYW will assist patients by integrating them back into the school system to ensure a successful transition. For patients involved with our outpatient mental health program who are struggling with socializing with their peers due to their mental illness, the CYW will engage them in social and recreational activities.
The CYW will work closely with the multidisciplinary teams to ensure that progress or delay in treatment is articulated to ensure a seamless transition between the teams. The CYW will also play an important role for patients who attend the Emergency Department in ensuring that the patient and their family are connected with in a timely fashion and supported until engaged to appropriate services.

- CYW certificate and diploma or degree from a recognized community college or university.
- Must have knowledge and skills in DBT and CBT
- Must have knowledge and skills in group design, development and implementation
- 3-5 years of child and adolescent experience preferably in the hospital setting
- Must have knowledge of children's psycho-social development
- Able to work independently and effectively within a team.
- Must have competency in assessing risk issues and being able to address concerns within the community and schools
- Experience in working with high conflict families and the skill set in deescalating conflict.
- Understanding of the school system and support within the Halton community.
- Must have computer skills (Word Processing)
- Satisfactory employment and good attendance record
Additional InformationHours: Monday to Friday, days/nights, 12 hour shifts (Hours are subject to change based on operational requirements)
Salary: $36.667 to $44.796 per hour
