Position SummaryReporting to Patient Education Manager, the Certified Diabetes Educator provides comprehensive diabetes self-management education and support to patients, helping them understand their condition, manage medications, adopt healthy lifestyle behaviors, and prevent complications. Using evidence-based guidelines, the educator conducts individualized assessments, develops tailored education plans, and collaborates with physicians and interdisciplinary care teams to optimize clinical outcomes.
We are seeking a Certified Diabetes Educator to serve as a clinical resource for diabetes management, promotes patient empowerment, and supports ongoing program development and quality improvement initiatives. This position plays a key role in supporting the Customer Success Team, ensuring alignment with organizational goals, operational excellence, and compliance standards.
Key Responsibilities- Provides individualized and group diabetes education, teaching patients evidence based self-management skills related to nutrition, medication use, glucose monitoring, and lifestyle modifications
- Conducts drug utilization reviews to evaluate medication adherence, appropriateness, dosing, and potential interactions, and collaborates with providers to optimize diabetes pharmacotherapy
- Delivers patient counseling on disease understanding, insulin and non-insulin therapies, injection techniques, and strategies for managing hypo /hyperglycemia, ensuring comprehension through teach back methods
- Serves as a clinical resource to providers and collaborates with interdisciplinary teams to improve patient outcomes and supports a unified approach to diabetes care.
- Ensures required courses, continuing education (CE), and compliance trainings are completed in accordance with state board requirements and company policy
- Partners cross-functionally to resolve complex patient issues, improve satisfaction metrics, and ensure exceptional patient experiences
Qualifications- Education: Bachelor's degree in nursing (Required)
- Licensure/Certification: Active nursing license and Certified Diabetes Care and Education Specialist (CDCES) Certification
- Experience:
- 3-5 years of experience as a diabetes educator (Required)
- 5+ years of experience as a diabetes educator (Preferred)
- Knowledge, Skills, & Abilities:
- Knowledge of evidence-based diabetes care; diabetes self-management education and support (DSMES) principles; pharmacotherapy for diabetes and comorbidities; nutrition, physical activity, and lifestyle counseling fundamentals; glucose monitoring technologies; and relevant regulations and documentation requirements (Required)
- Knowledge of advanced device therapies; pediatric, gestational diabetes, older adults, renal impairment, and/or complex cardiometabolic diseases; and behavioral integration pathways (Preferred)
- Clinical assessment, patient education, and health literacy skills (Required)
- Excellent written, verbal, and interpersonal communications skills (Required)
- Medication management support and documentation skills (Required)
- Program development, quality improvement, and data management skills (Preferred)
- Ability to triage, escalating urgent concerns where needed and to collaborate with the care team (Required)
- Ability to facilitate group classes and workshops and develop individualized education plans (Required)
- Ability to serve as a clinical resource and drive medication access solutions (Required)
Work Environment- Location: On-site
- Schedule: Availability from 8:00 A.M. to 8:00 P.M. Monday through Friday with night and weekend hours on occasion as determined by the needs of the business and cross-facility travel as needed
- Regular meetings with internal Patient Education, Customer Success, Pharmacy Operations, and senior leadership teams. This role may also have meetings with external vendor and consultant partner representatives
Key Essential Functions- Must be able to remain in a stationary position for extended periods while writing or reviewing documentation
- Must be able to work on a computer for the entire shift
- Must be able to attend virtual meetings with cross-functional teams
