Short DescriptionThe case manager provides crisis intervention, consultation, referrals, and brief interventions to patients. The role also completes follow-up with patients to assist in treatment linkages and navigating complex social service applications such as housing applications, disability enrollments, etc.
Experience RequiredPrevious acute care experience preferred.
Education RequirementsBachelor Degree -Social Work/Registered Nurse required.
License/Certification Requirements- RN licensure in the State of New Jersey.
