Practice Details- Majority of this role is focused on clinic with small portion of dialysis or hospital rounding

Qualifications- Licensed Nurse Practitioner or Physician Assistant in California
- Bilingual in Spanish and English strongly preferred
- Excellent communication and patient education skills
- Interest in chronic disease management and specialty care

About the RoleThis provider will work closely with nephrologists and interdisciplinary care teams to evaluate, diagnose, and manage patients with chronic kidney disease, hypertension, dialysis needs, and related medical conditions.
Responsibilities include:
- Conducting patient assessments and physical examinations
- Developing treatment plans
- Managing medications and chronic conditions
- Documenting care accurately within the EMR
- Educating patients and families on treatment plans and disease management
This role has a salary range of $130,000 - $140,000 and regular, full-time employees working 30 or more hours per week are eligible for comprehensive benefits including Medical, Dental, Vision, Life, 401(K), Paid time off (PTO).
