The Bilingual Care Manager, Telephonic Nurse 2, in a telephonic environment, assesses and evaluates members' needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Care Manager, Telephonic Nurse 2 work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action.
The Bilingual Care Manager, Telephonic Nurse 2 employs a variety of strategies, approaches and techniques to manage a member's physical, environmental and psycho-social health issues. Identifies and resolves barriers that hinder effective care. Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through use of assessment, data, conversations with member, and active care planning. Must be able to navigate multiple systems and use critical thinking skills.
Required Qualifications- Licensed Registered Nurse (RN) in the state of Illinois with no disciplinary action or ability to obtain IL RN licensure.
- Minimum of 3 years of clinical acute care experience as a licensed RN.
- Bachelor's degree in a health-related field.
- Bilingual in English/Spanish Must be able to speak, read, and write in both languages without limitations or assistance. See Additional Information on testing.
Preferred Qualifications- Bachelor's degree in Nursing (BSN).
- Experience with case management, discharge planning and patient education for adult acute care.
- Managed care experience.
- Certified Case Manager (CCM).
- Prior experience with Medicare & Medicaid recipients.
- Experience with health promotion, coaching and wellness.
- Knowledge of community health and social service agencies and additional community resources.

- Typical work days/hours: Monday - Friday, 8:00am - 5:00pm CST, occasional flexibility in scheduling may be required to accommodate member needs.
Language Proficiency TestingAny Humana associate who speaks with a member in a language other than English must take a language proficiency assessment, provided by an outside vendor, to ensure competency. Applicants will be required to take the Interagency Language Rating (ILR) test as provided by the Federal Government.

In certain roles, the minimum recommended internet speed required by Humana may not be sufficient for business needs. Humana reserves the right to require associates to upgrade their internet service if necessary. Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information.
Travel: While this is a remote position, occasional travel to Humana's offices for training or meetings may be required.

Pay RangeThe compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
$71,100 - $97,800 per year
This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
