Department:Job Description Summary:Position Responsibilities- Develop, support and manages clinical pharmacy related programs and services across all departments of Mount Sinai Medical Center
- Responsible for managing and monitoring the hospital drug formulary: completing drug class reviews, minimizing non-formulary procurements, suggesting appropriate, cost-effective therapeutic alternatives to medical staff, and implementing automatic therapeutic substitutions when appropriate.
- Identify, design, analyze and report drug utilization reviews to promote rational and safe drug therapies and determines action plan based on results.
- Train and participate with Disaster Team drills and real-event responses as assigned. Participate in quality and regulatory reviews and audits.
- Work with the Pharmacy leadership to identify and develop processes to meet or exceed all regulatory standards.
- Act as manager in charge as assigned. Effectively and proactively intervene in conflicts between pharmacy and medical staff regarding drug therapy issue resolutions.
- Work with Pharmacy leadership to establish goals and objectives of clinical pharmacy programs and responsible for implementation and monitoring within budgetary targets.
- Participate with various nursing and medical staff committees, as necessary. Coordinates all activities related to the Pharmacy and Therapeutics Committee, including the publication of its newsletter.
- Supports the coordination of the pharmacy review of medication orders in physician order sets, protocols or other evidenced based medication use guidelines across the organization
- In conjunction with Pharmacy Management team, responsible for the development and review of clinical pharmacy policies and procedures.
- Maximizes area's impact by ensuring adequate staffing levels and the most efficient organization and use of resources are available to support all clinical programs.
- Responsible to develop and actively participate in training and mentorship programs for pharmacists, pharmacy students, residents, technicians, nurses and physicians as it relates to clinical pharmacy services.
- Ongoing analysis and reporting of clinical pharmacy interventions to ensure accurate, high quality and appropriate documentation is being completed
- Builds effective relationships with other healthcare professionals and departments within the medical center.
- Defines the team's target accountabilities as well as the guidelines and practices to accomplish them.
- Ensures the implementation and completion of the teams' regular and special projects as assigned.
- Responsible for the completion of Performance Evaluations with timely and constructive feedback for the assigned team.
- Follows hospital's policies and procedures regarding Human Resources Management.
- Ensures that new employees complete general and department-specific orientation as set forth into the hospital's policies and procedures. Documents all personnel actions and submits them immediately for review.
- Guides and directs the team efforts. Ensures that the team has the necessary resources to perform as expected.
- Analyzes the team's outcomes with ongoing assessment of established goals. Identifies areas of opportunity and respective action plans.
- Ensures that all procedures are carried out in accordance with company policy.
- Ensures that customers receive standards of service in line with the policy and strategy.
- Supports the day to day activities of the staff.
- Follows the National Patient Safety goals as required:
- Improve accuracy of patient identification with at least two identifiers (excluding patient's room number).
- Reduce the risk of health care-associated infections.
- Reduce the risk of patient harm resulting from falls.
- Follows Patient Safety goals as established by the department; supports effective responses to adverse and sentinel events; provides an on-going proactive approach to reduce risk; integrates patient safety as a priority into new and existing processes.
- Is available to participate as a member of either Team A or Team B of the Hurricane Activation Plan, as assigned.
- Reports incidents to the Director and/or Risk Management department within required timeframe in accordance with medical center policy.
- Complies with current Hospital security and safety polices.
- All other tasks as assigned.
Qualifications- License/Registration/Certification
- Licensed as a Pharmacist in the State of Florida.
- Education
- Graduate from an accredited School of Pharmacy. Masters Degree in Business or Healthcare Administration related fields preferred.
- Experience
- Eight+ years in Pharmacy Management comparable role.
