Job Description Summary

The Area Director of Clinical Operations- Home Health is a professional, Registered Nurse or Physical Therapist responsible for all of the quality activities of the organization. The Area Director of Clinical Operations has accountabilities for the monitoring, implementation and evaluation of process and programs to meet regulatory, accreditation and standards of practice.

Essential Job Functions/Responsibilities
  1. Develops and promotes programs for staff support and in-services for all interdisciplinary team members at local offices
  2. Assesses staff learning needs and educates the interdisciplinary team members on all clinical aspects of the hospice program.
  3. Assesses staff learning needs and educates the interdisciplinary team members on all clinical best practices, new policies, and procedures.
  4. Develop, recommend, and evaluate policies and procedures relevant to provision of health services and monitors compliance
  5. Conducts internal quality checks, identify areas in need of improvement and make recommendations to ensure patients/ families receive high quality hospice care.
  6. Develops and provides home health training for executive directors, Director of patient care services, and clinical managers to encourage standardization and collaboration among home health providers.
  7. Develops and provides in-service training as needed
    1. Establishes a process model for improvement and provides education and training to organization personnel on the performance improvement plan, as well as team development.
  8. Survey support
  9. Develops and initiates organization mock surveys, to establish process breaks and needed areas of improvement
  10. Develop, implement, and maintain policies and procedures related to the agency QAPI and compliance program.
  11. Acts as a clinical resource to personnel.
    a) Maintains updated nursing knowledge and skills.
  12. Provides assistance to community education team in meeting community education needs.

    b) Acts as a representative of the organization at community meetings, civic and professional organizations as needed.
  2. Promotes self-growth and collegial relationships with others in the home health industry.
    A. Maintains professional and technical knowledge by attending educational workshops, reviewing professional publications, establishing personal and professional networks and participates in professional organizations or societies.

Job Qualifications

  • Registered Nurse or Physical Therapist with current licensure to practice professional nursing/ physical therapy in the state.
  • Bachelor's degree in nursing from an accredited program by the National League for Nursing (preferred). Master's degree or above in Physical Therapy.
  • Three (3) to five (5) year's clinical nursing/ therapy experience, in a Medicare Certified home health.
  • Must have a valid Driver's License with state minimum insurance coverage.
  • Excellent understanding of performance improvement, quality assurance, Value Based Purchasing and Star Measures and utilization management.
  • Is self-directed and able to work with minimal supervision.
  • Has excellent time management, communication, and problem-solving skills.
  • Proficient in Oasis- ( COSC or HCS-O preferred)
  • HCHB EMR experience preferred.
  • Willing to travel up to 50% of the time.
