Advanced Practice Provider (APP) – Outpatient

Position Summary

Heart South is seeking a dedicated Advanced Practice Provider (APP)—Nurse Practitioner (NP) or Physician Assistant (PA)—to join our outpatient care team. This role provides comprehensive patient care in a non-surgical outpatient setting, including assessment, diagnosis, treatment, and patient education, while working collaboratively with physicians in a team-based care model.

 

This is an excellent opportunity for an APP who values continuity of care, patient education, and working in a supportive outpatient clinic environment.

 

Responsibilities

Key Responsibilities

  • Provide comprehensive outpatient care, including patient assessments, diagnosis, and treatment planning
  • Perform physical exams, review diagnostic studies, and interpret laboratory results
  • Develop and implement individualized treatment and follow-up plans
  • Document patient care, assessments, and plans accurately and timely in the electronic medical record
  • Provide preventive care and health education to patients and families
  • Triage patient needs and manage care issues to improve patient outcomes
  • Collaborate closely with physicians and other healthcare disciplines to ensure quality and continuity of care
  • Serve as an advanced clinical resource within the clinic or specialty area
  • Demonstrate compassionate, patient-centered care across all age groups
  • Participate in weekend, holiday, or on-call duties as required by the service line
  • Maintain compliance with all organizational, regulatory, and accrediting standards
Qualifications

Education & Training

  • Nurse Practitioner: MN, MSN, DNP, PhD, or DNS degree in NursingOR
  • Physician Assistant: Graduate of an accredited Physician Assistant program

Licensure & Certifications

  • Must hold one of the following:
    • Alabama ARNP license with national advanced certification
    • Current Alabama Physician Assistant license
  • Current BLS certification required

Experience

  • No prior experience required
