Duly Health and Care Lombard is seeking an APP who is committed to empowering health and elevating care for our patients while also pursuing innovative solutions with their care team that move healthcare forward. This role includes a combination of outpatient clinic visits and dialysis rounding, providing comprehensive care to patients with chronic kidney disease and related conditions.
Key Responsibilities:- Provide high-quality, evidence-based care to Urology/Uro-Oncology patients in both clinic settings.
- Conduct thorough patient assessments, develop care plans, and manage ongoing treatment in collaboration with supervising physicians.
- Perform dialysis rounding, including evaluating patients, managing medications, and coordinating care with dialysis center staff.
- Document all patient encounters and care plans accurately and efficiently in epic.
- Communicate clearly with patients, families, clinic staff, and interdisciplinary teams.
- Support nephrology providers by assisting with patient follow-up, education, and chronic disease management.
- Participate in quality initiatives, care coordination, and process improvement efforts.

Compensation & BenefitsThe anticipated base compensation range for this position is
100,000-140,000, depending upon the applicant's qualifications and experience. In addition to the base compensation, this position may also be eligible for a sign-on bonus.

Qualifications-- Graduation from an approved school for Physician Assistant Or Nurse Practitioner-Masters Degree.
- Licensed or ability to obtain licensure in the state of Illinois
- Previous experience (1-2 years) in Urology/Uro-Oncology or similar setting preferred.
