Job Description Summary:

APP Director of Primary Care for all RMR Regions

$75 per hour for 20 hours weekly ($78,000) + base 0.5 FTE of $81,402.50
total Annual salary of $159,402.50

The APP Director is responsible for providing direction for the development and clinical performance of the Primary Care Network ("PCN") within the Primary Health Division ("PHD") and advise AdventHealth ("A_H") on all operational and professional matters relating to the role and function of this position. APP Director is expected to monitor and require the highest standards of professionalism of APP Provider") colleagues at all times. Duties include but are not limited to:

Job Description:

Responsibility for hosting critical and collaborative conversations with PHD Primary Care ("PCP") APPs, and office staff to improve professionalism and performance in clinical practice.
  • Demonstrate respect for patients, colleagues, and staff.
  • Maintain confidentiality of patient and clinic information.
  • Uphold ethical and evidence-based standards in all aspects of patient care.
  • Participate in ongoing professional development activities to enhance clinical productivity, skills, and knowledge.
  • Maintains current knowledge of CMS regulations and new care delivery models and assists in leading clinical operational strategy and identifies appropriate education needed for APPs and offices to perform optimally.
  • Leads the strategic development, implementation and maintenance of strong, comprehensive clinical programs that meet APP growth initiatives, new care delivery models and the needs of patient populations in the community.
  • Responsible for APP coaching and engagement strategies including APP recruitment and promoting programs which enhance APP satisfaction. Actively engage in efforts to recruit APPs who will support the mission, vision, values and purpose of PCN and AH.
  • Provides leadership, supervision, guidance and development for APPs, articulating and demonstrating an expectation for continuous quality improvement, as well as continually supporting and exhibiting AH service standards.
  • Collaborates with the clinical leadership within PHD-PCP and the operational leadership to ensure that appropriate APP policies and procedures are operational to ensure excellent clinical performance.
  • Develops and leads APP growth, professional development and assessment of performance.
  • Develops and leads in collaboration with leadership and physician teams strategies that optimize PHC-PCP as a preferred employer of APPs.
  • Ensures adherence to APP practices on professionalism, quality and safety issues, and excessive or inappropriate utilization of resources.
  • Responsible for hosting critical and collaborative conversations with PHD-PCP APPs, and office staff to improve quality outcomes and performance in all payor contracts.

  • Stay up to date with all state requirements for APP credentialing, including registering a supervising physician, developing scope of work protocols, required CME, etc.

It is understood that the APP Director may not be able to, in any one month, perform all of the aforementioned functions. This Agreement represents all of the Services to be provided by the APP Director to PCN. These Services do not exceed those that are reasonable and necessary for the legitimate business purpose of the arrangement.

II.TERM AND TERMINATION :

APP Director duties shall commence as of the Effective Date of this Addendum for APP Director Services and shall continue throughout the term of APP Employer, unless earlier terminated as provided herein.

This Addendum for APP Director Services may be terminated at any time by any undersigned party hereto upon sixty (60) days prior written notice to the other undersigned parties. In the event this Addendum is so terminated, the undersigned parties may not enter into another agreement for the same or similar services until one (1) year after the Effective Date of this Addendum. In the event that the Employment Agreement between Employer and APP is terminated for any reason whatsoever, and notwithstanding anything to the contrary set forth herein, this Addendum for APP Director Services shall terminate, with no liability to Employer upon such termination. The effective date of termination of the Agreement shall be coterminous with the Termination Date of APP employment with Employer.
