On-Call Advanced Practice Provider (Nurse Practitioner) to provide after-hours telehealth support for our Institutional Special Needs Plan (I-SNP) and long-term care population.
This role is critical to ensuring residents receive timely, high-quality care while avoiding unnecessary emergency department visits and hospital admissions. Working collaboratively with nursing facility staff and our clinical teams, you will assess acute changes in condition, initiate treatment plans, and support a proactive treat-in-place model of care.
WHAT YOU'LL DOProvide Acute Clinical Assessment & Intervention- Conduct after-hours telehealth evaluations via phone and video for urgent and emergent resident concerns.
- Assess acute changes in condition using evidence-based clinical guidelines.
- Order and manage diagnostics, treatments, and interventions as clinically indicated.
- Initiate early interventions for common long-term care conditions, including:
- Infections
- Dehydration
- COPD/Asthma Exacerbations
- Heart Failure Symptoms
- Behavioral Changes
- Pain Crises
Help Reduce Avoidable Hospital Transfers- Utilize strong clinical judgment to stabilize residents in place whenever appropriate.
- Collaborate with facility teams to coordinate treatments such as IV therapy, hydration protocols, medication adjustments, wound care, and respiratory therapies.
- Document clinical rationale and decision-making when hospital transfer is required.
Support Chronic Disease Management- Manage after-hours exacerbations of chronic conditions including:
- CHF
- COPD
- Diabetes
- Dementia
- Behavioral Health Conditions
- Adjust medications and reinforce individualized care plans to support resident stability and reduce hospitalizations.
Coordinate Care & Documentation- Complete accurate and timely EMR documentation.
- Communicate handoff summaries and follow-up needs to the daytime clinical team.
- Identify residents requiring urgent next-day evaluation.
- Assist with onboarding and documentation of new member information, including demographics, medications, care team information, vaccinations, and advance care planning documentation.
RequirementsWHAT WE'RE LOOKING FORRequired Qualifications- Master of Science in Nursing (MSN) from an accredited Nurse Practitioner program.
- Current unrestricted Nurse Practitioner license.
- National board certification (AANP, ANCC, AACN, or equivalent).
- Board certification as an Adult, Family, Geriatric, or Primary Care Nurse Practitioner.
- DEA license.
- Eligible to participate in Medicare as a provider.
- Minimum of two (2) years of Advanced Practice Provider experience.
- Minimum of one (1) year of experience working in a nursing home or long-term care setting.
- Minimum of one (1) year of after-hours telehealth experience or I-SNP experience.
- Strong computer and technology proficiency.
- Excellent communication, clinical assessment, critical thinking, and organizational skills.
Licensure RequirementsAPP licensure and credentialing will be required in all active program states, currently including:
- Texas
- Florida
- Colorado
- Michigan
- North Carolina
- South Carolina
Providers will be required to initiate licensure applications in all active states within 30 days of hire. Additional state licensure requirements may be added as the program expands.
