Job Description

Functions as patient facing coordinator for the Northwell MAID (Medical Aid in Dying Program). Provides strategic advice and guidance to Social Work department. Provides professional input to ensure completion of departmental goals, compatible with overall needs & policies. Participates in the training and development of Social Workers.

Job Responsibility
  • Serves as the first point of entry for all MAID-related inquiries from patients, families, hospital staff, providers, and external stakeholders.
  • Conducts detailed psychosocial assessments and oversees appointments and eligibility verification
  • Collaborates closely with patients and their families to address all practical and logistical considerations regarding MAID
  • Provides supportive counseling and appropriate referrals to patients and their families throughout the MAID process.
  • Contributes to the ongoing development and implementation of Northwell's MAID program, ensuring adherence to regulatory standards, ethical considerations, and best practices.
  • May collect key metrics and data for tracking and quality purposes.

1.Provides social work strategic advice and guidance to functional areas and assures educational adherence to social work guidelines, policies, and procedures.
2.Works with senior executives to develop and drive social work system-wide functional strategies and guide site-based learning and performance across the continuum to social work employees, clinicians, and site leaders.
3.Provides guidance to social work team members from basic to complex tasks and projects. Responsible for the review, development and oversite of system social work orientation program and competencies.
4.As an advanced subject matter expert in the social work profession, works at very high levels in multiple knowledge and functional areas across the system.
5.Regularly provides guidance to others in the use of new technologies, clinical theories, transition of care concepts and general social work techniques.
6.Frequently serves as social work technical advisor to management.
7.Serves as project/technical lead on small to medium sized projects.
8.May provide input into performance reviews for team members.
9.Serves as a clinical process leader; applies extensive knowledge of social work skills, trends, and clinical practice to healthcare settings
10.Interprets complicated and complex patient care needs and strategizes necessary trainings and education to manage effectively
11.Collaborates closely with senior executives and business team in modifying social work-related patient care approaches/processes to ensure safe patient transitions, effective interventions, and patient satisfaction.

Job Qualification

1.Master of Social Work required.
2.Current License to practice as a Clinical Social Worker in New York State required, plus specialized certifications as needed.
3.10+ years of technical experience and 2+ years of leadership / management experience, required.

Highly Preferred:

1. Experience working with individuals and families facing life-limiting illnesses, end-of-life decisions, and bereavement.
2. Solid grasp of healthcare ethics, including patient autonomy, informed consent, confidentiality, and professional boundaries.
3. Experience in program development

Hybrid- travel to all regions/sites as needed.
