Full Job Description
POSITION SUMMARY:

The Advanced Practitioner (APP) is responsible for providing necessary and appropriate outpatient Primary Care services and Extended Care Center Services within the scope of the Provider's training and experience. The APP will function independently to conduct all aspects of a medical evaluation to include eliciting pertinent medical information, performing physical examinations, order appropriate testing, and formulating an assessment and plan for each patient encounter. The APP will also be responsible for offering patient education, accurately documenting all encounters in the electronic health record (EHR) within 24hrs of the encounter, and responding to patient questions and messages. The APP will be responsible for reaching out and conferring with the supervising/collaborating physician in cases where there are questions or concerns about medical care.

QUALIFICATIONS:
• Advanced Practitioner licensure
• Minimum 3 years' post licensing experience, working independently, in family medicine, urgent care clinics or a combination
• Graduate of an accredited program, APRN or PA, program
• Computer skills and experience with Microsoft Office programs
• Electronic Medical Records (EMR) experience (good typing & computer skills)
• Eligibility to work in the United States.
• Clear English communication skills both verbal and written are necessary to effectively communicate with various teams, departments, other health facilities.
• Current Driver's License
• Basic Life Support (BLS) certification
