Position Summary

The Physician Group of Capital Health Plan is seeking a Nurse Practitioner or Physician Assistant to support a busy family practice physician group through acute and chronic care appointments and assistance with electronic health record inbox management. This role is designed to improve patient access and strengthen care coordination.

Key Responsibilities
  • Evaluate and treat patients during acute care, same-day, and short-notice appointments, as well as routine follow-up visits for chronic conditions.
  • Document patient encounters, clinical decisions, communications, and follow-up plans accurately and timely in the electronic health record.
  • Collaborate closely with physicians, registered nurses, licensed practical nurses, medical assistants, reception staff, and other members of the care team to support continuity of care.
  • Identify patients who require physician review, escalation, emergency evaluation, specialty referral, or additional follow-up.
  • Assist with electronic health record inbox tasks for assigned family practice physicians, including patient messages, clinical questions, laboratory and imaging results, refill requests, forms, and follow-up needs.
  • Support quality improvement initiatives, access goals, and patient satisfaction efforts within the primary care practice.

Qualifications
  • Current licensure as an Advanced Practice Registered Nurse or Physician Assistant in the State of Florida, or eligibility to obtain Florida licensure.
  • Graduate of an accredited Nurse Practitioner or Physician Assistant program.
  • National certification appropriate to training and specialty.
  • Family medicine or urgent care experience preferred.
  • Comfort using an electronic health record to manage patient communications, results, refill workflows, documentation, and team-based care tasks.
  • Strong clinical judgment, communication skills, organizational ability, and commitment to patient-centered care.
  • Ability to work independently within scope of practice while collaborating effectively with supervising or collaborating physicians and the broader care team.
