Overview:

Sentara Norfolk General Hospital has a full-time APP to join Sentara's Case Management Pharmacy Program, VA. Work from home or at the hospital! Remote work available. Our Advanced Practice Providers (APPs) are essential to this foundation, and we seek to continue to grow their impact and scope of practice throughout our hospitals.

Position Highlights:
• Join an experienced team of 1 MD, 9 nurse case managers, 5 clinical pharmacists, and 1 nurse supervisor
• Primary Location: Sentara Norfolk General Hospital
• Schedule: Mon - Fri, 8:30AM - 5:00PM, no weekends or holidays and working from home is available
• Duties: Perform health risk assessments, re-write prescriptions
• Review 150 patients per month
• Work with Sentara Medicaid only
• 1 year experience required as APP in Family Medicine
• Experience in Rheumatology or Infectious Disease preferred

We provide market-competitive compensation packages, inclusive of base pay, incentives, and benefits. The base pay range for full-time employment is $100,900.80 - $312,000.00 annually.Additional compensation may be available for this role, such as shift differentials, standby/on-call pay, overtime, shift premiums, extra-shift incentives, or bonus opportunities. Compensation within the range may vary based on qualifications, experience, location, market conditions, and business needs. Note: If an annual salary is posted, it is based on a full-time colleague working 2,080 hours annually.
