Job Location

Community Physician Network seeks a Nurse Practitioner or Physician Assistant to join our Palliative Care team in Anderson, IN. The most comprehensive palliative care program of its kind in Indiana, our team is committed to helping patients and families understand where they are on their medical journey and providing the physical and emotional comfort they need.
  • 1.0 FTE position, weekdays
  • This provider will work autonomously and collaboratively with our palliative care multi-disciplinary team at Community Hospital Anderson to deliver palliative and health care services to patients.
  • Focus on the comfort measures of patients, end of life care, symptom management, as well as assisting with consults and new admissions.
  • Flexibility to travel when needed

Qualifications

Ideal candidates will:
  • Possess 1-3 years' Experience as a licensed Nurse Practitioner in Palliative Care
  • Be comfortable assessing patients independently
  • Be compassionate caregivers with excellent organizational, communication, and critical thinking skills
  • Possess highly developed interpersonal and group dynamic skills, as well as proven skills as a consultant/educator to friends, families, and loved ones
Licensure & Certifications

Educational background consistent with requirements for licensure and certification within the state of Indiana

Community Health Systems is a Fortune 500 company based in Franklin, Tennessee. It was the largest provider of general hospital healthcare services in the United States in terms of number of acute care facilities.
