The
Medical College of Wisconsin's Department of Medicine, Division of Nephrology is seeking a
Nurse Practitioner or Physician Assistant to provide primary or specialty medical care within defined scope of practice under the supervision of a licensed physician and in accordance with MCW/FMLH/CHW policies and procedures. This position is in
Milwaukee, WI at MCW/Froedtert Health.The APP will have a strong foundation in evidence-based practice and ensure an environment of safe quality care.
In this role you will be responsible for the following:- Collaborates with interdisciplinary care teams to evaluate and manage kidney transplant recipients in the inpatient setting as a member of the nephrology transplant team.
- Provides specialty care for kidney transplant recipients, including evaluation, treatment planning, and ongoing management.
- Performs approved privileged procedures in accordance with nephrology service requirements.
- Facilitates communication among patients, families, and care team members to support continuity of care throughout hospitalization and transitions of care.
- Participate in quality improvement initiatives, including development and evaluation of quality indicators and outcome measures.
- Completes required training and maintain compliance with institutional, CMS, Joint Commission, and privileging requirements.
- Maintains current clinical knowledge and specialty expertise through continuing education, professional organizations, and review of relevant literature.
Core Advanced Practice Responsibilities:- Performs comprehensive patient assessments, including history, physical examination, and evaluation of physical, cognitive, and psychosocial status.
- Orders, performs, and interprets laboratory, diagnostic, and screening tests.
- Develops, implements, and evaluates pharmacologic and nonpharmacologic treatment plans.
- Monitors patient progress and modifies the plan of care as clinically appropriate.
- Performs procedures within approved scope of practice and credentialing guidelines.
- Documents patient care activities accurately and timely within the electronic medical record (EMR).
- Educates and counsels patients and families regarding diagnoses, treatment plans, preventive care, and health maintenance.
- Collaborates and consults with physicians and other health care professionals to coordinate patient care.
- Apply clinical judgment and evidence-based practice to support decision-making and management of complex patient care situations.
- Prioritizes and manages clinical responsibilities effectively to meet patient care needs and organizational expectations.
- Communicates clearly and professionally in verbal and written formats.
- Establishes and maintains effective working relationships with patients, families, providers, and staff.
- Utilizes organizational and community resources to support optimal patient outcomes.
- Maintains professional competence through ongoing learning, self-assessment, and incorporation of current research and best practices.
- Demonstrates professionalism, ethical conduct, accountability, patient confidentiality, and adherence to applicable policies, regulatory requirements, and standards of practice.

Preferred Schedule: Full-time role with expectations for coverage during core business hours and flexibility required as necessary to accommodate business needs. Evening or weekend hours in the future based on business needs.
Position Requirements:Preferred Experience: Preference will be given to practitioners with prior experience in hospital-based care and /or medical/surgical patients.
Minimum Education:Advanced Practice Nurse: Master's Degree or higher in Nursing
Physician Assistant: Bachelor's Degree or higher in Physician Assistant Studies
Minimum Requirements: *or eligible
Advanced Practice Regsistered Nurse - Wisconsin RN license
- Wisconsin Advance Practice Nurse Prescriber License
- Certification as Advance Practice Nurse
- Certification as Adult Care NP and/or Acute Care NP preferred
- AHA BCLS
- CITI Certification to be completed within 90 days of hire
- ACLS Certification required or to be completed within 90 days of hire
