Full Job Description
Central Ohio Primary Care is seeking a full-time Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join Family Practice Center of Westerville.
The APP will work collaboratively with physicians and clinical staff to provide high-quality, patient-centered primary care for patients of all ages. Responsibilities include assessment, diagnosis, treatment planning, chronic disease management, preventive care, and patient education in a busy outpatient family medicine setting.

Key Responsibilities
  • Perform comprehensive patient evaluations including medical histories, physical examinations, and diagnostic assessments
  • Diagnose and manage acute and chronic medical conditions across the lifespan
  • Provide preventive care services including annual wellness visits, screenings, immunizations, and health counseling
  • Develop and manage treatment plans for chronic diseases such as diabetes, hypertension, asthma, COPD, and hyperlipidemia
  • Prescribe medications and monitor treatment effectiveness in accordance with Ohio licensure and practice guidelines
  • Educate patients and families on disease prevention, treatment options, medications, and healthy lifestyle strategies
  • Collaborate with physicians, nursing staff, and specialists to coordinate comprehensive patient care
  • Facilitate referrals, follow-up care, and transitions of care as needed
  • Maintain accurate and timely documentation within the electronic health record system (EPIC)
