Central Ohio Primary Care is seeking a full-time Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join Family Practice Center of Westerville.
The APP will work collaboratively with physicians and clinical staff to provide high-quality, patient-centered primary care for patients of all ages. Responsibilities include assessment, diagnosis, treatment planning, chronic disease management, preventive care, and patient education in a busy outpatient family medicine setting.
Key Responsibilities- Perform comprehensive patient evaluations including medical histories, physical examinations, and diagnostic assessments
- Diagnose and manage acute and chronic medical conditions across the lifespan
- Provide preventive care services including annual wellness visits, screenings, immunizations, and health counseling
- Develop and manage treatment plans for chronic diseases such as diabetes, hypertension, asthma, COPD, and hyperlipidemia
- Prescribe medications and monitor treatment effectiveness in accordance with Ohio licensure and practice guidelines
- Educate patients and families on disease prevention, treatment options, medications, and healthy lifestyle strategies
- Collaborate with physicians, nursing staff, and specialists to coordinate comprehensive patient care
- Facilitate referrals, follow-up care, and transitions of care as needed
- Maintain accurate and timely documentation within the electronic health record system (EPIC)
Qualifications- Master's degree from an accredited Nurse Practitioner or Physician Assistant program.
- Current Ohio licensure as a Nurse Practitioner or Physician Assistant.
- National certification as a CNP or PA-C.
- Minimum of one year of post-graduate clinical experience preferred, ideally in outpatient primary care or family medicine.
- Strong clinical assessment, diagnostic, and patient management skills.
- Excellent communication and interpersonal abilities.
- Ability to work independently while collaborating effectively within a multidisciplinary team.
- Experience with EPIC EHR preferred, though training will be provided.
