Description
Position Summary:Provides a full range of primary care services consistent with the APP's scope of practice under applicable state law and the general direction of the Chief Medical Officer. Manages an assigned patient panel, collaborates with the provider and support team, participates in clinical quality and peer review activities, and supports implementation of clinical guidelines and organizational care initiatives.
Essential Duties and Responsibilities:The following duties represent the essential functions of the position.
- Works effectively with the Chief Medical Officer and other members of the care team to provide high-quality medical care in a team-based environment and manages an assigned panel of patients.
- Provides a full range of primary care services in the outpatient setting and coordinates referrals to outside services when appropriate.
- Assesses, diagnoses, and treats acute and chronic health conditions within the approved scope of practice.
- Coordinates referrals to specialists and other services when appropriate.
- Participates in meaningful peer review processes and meets applicable quality measure and documentation requirements.
- Works respectfully and effectively with executive and leadership personnel in support of the MLMC mission, strategic plan, and organizational goals.
- Communicates timely through appropriate channels to support team-based, patient-centered care.
- Participates in Provider Meetings, Town Hall meetings, committees, and team responsibilities as assigned, except when on approved leave.
- Participates in community outreach activities, including health fairs, school-based clinics, educational programs, and similar community functions, and promotes MLMC services to the community.
- Collaborates with MLMC staff and services in comprehensive patient treatment, disease prevention, and continuity of care.
- Supports grant and funding initiatives consistent with the MLMC mission and participates in activities needed to accomplish related goals and objectives.
- Complies with established clinical policies and procedures and standards of medical care.
- Participates in MLMC programs and contractual arrangements that promote quality, team-based, and patient-centered care, including value-based care initiatives as applicable.
- Completes charts and patient care documentation within established MLMC timeframes and protocols.
- Addresses patient messages, diagnostic results, and laboratory results through the electronic health record in a timely manner.
- Provides courteous, compassionate, and patient-centered service to employees and patients.
- Effectively manages multiple tasks and priorities.
- Complies with applicable terms of the Employment Agreement.
- Performs other duties as assigned by the supervisor.

Requirements
QualificationsQualifications include the knowledge, skills, education, experience, and credentials necessary to perform the essential functions of the position satisfactorily.
Required:- Master's degree from an accredited Nurse Practitioner or Physician Assistant program.
- Current and unrestricted license to practice as a Nurse Practitioner or Physician Assistant in each applicable state.
- Current national board certification applicable to the provider's discipline and specialty.
- Current Basic Life Support (BLS) certification, or willingness to obtain within three months of hire.
- Valid government-issued photo identification.
- Valid driver's license and acceptable driving record.
- Ability to travel locally and occasionally out of state as required.
- Experience using electronic health records.
- Ability to provide clinically competent care within the approved scope of practice.
- Ability to establish effective working relationships with patients, employees, healthcare professionals, and community partners.
Preferred- Minimum of three years experience providing primary care services.
- Current Advanced Cardiovascular Life Support (ACLS), Advanced Trauma Life Support (ATLS), or Pediatric Advanced Life Support (PALS) certification.
- Experience in an FQHC, community health center, or setting serving underserved populations.

Salary Description
$95,000 to $130,000 Annually
