Advanced Practice Provider (NP/PA)

Heart Valve Center - St. Francis Hospital (SFH)

Inpatient and Outpatient

FT - 4 X 10 hr. shifts

Position Summary
The Advanced Practice Provider (Nurse Practitioner or Physician Assistant) will support the Heart Valve Center at St. Francis Hospital, providing comprehensive, patient-centered care to individuals with complex valvular heart disease. The APP will work collaboratively with cardiologists, cardiothoracic surgeons, structural heart specialists, nursing, and multidisciplinary teams to ensure seamless evaluation, treatment planning, procedural coordination, and longitudinal follow-up care.

This role is integral to delivering high-quality cardiovascular outcomes and enhancing the patient experience throughout the continuum of care.

Key Responsibilities
  • Key Responsibilities
    • Conduct comprehensive cardiovascular assessments for patients with valvular heart disease.
    • Participate in pre-procedure evaluation and optimization for TAVR, TMVR, TTVR mitral and tricuspid TEER, and other structural heart interventions.
    • Provide inpatient and outpatient management, including post-procedural follow-up.
    • Collaborate with the multidisciplinary Heart Team in treatment planning and case review conferences.
    • Educate patients and families regarding diagnosis, treatment options, and post-discharge care.
    • Coordinate care transitions to ensure continuity across hospital and outpatient settings.
    • Document clinical findings and treatment plans in accordance with regulatory and institutional standards.
    • Support quality improvement initiatives and data tracking related to valve program metrics.
