Qualifications

Responsibilities

  • Work Monday to Friday from 8 AM to 5 PM with no call responsibility
  • Engage in a rotating outpatient/inpatient schedule to broaden skill sets
  • Manage adult-only patients with a focus on heart failure
  • Deliver comprehensive care, including diagnosis and treatment planning
  • Act as the primary contact for patients with chronic cardiovascular issues

Responsibilities

Job Title: Advanced Practice Provider - Heart Failure

Location: Lebanon, PA

Employment Type: Full-Time

What You'll Champion:
  • M-F 8 AM - 5 PM schedule. No call responsibility
  • Rotating outpatient/inpatient schedule for variety and skill development
  • Adult-only cardiology patients with a focus on heart failure management
  • Provide comprehensive care for heart failure patients including diagnosis, medication management, treatment planning, and patient education
  • Serve as a primary point of contact for patients with chronic cardiovascular conditions

Qualifications

Minimum Education:
  • Masters Degree Nurse Practitioner Required or

Work Experience:
  • 1 year Required

Licenses:
  • Certified Registered Nurse Practitioner Upon Hire Required or

  • Basic Life Support Upon Hire Required or
  • Advanced Cardiac Life Support Upon Hire Required

Courses and Training:
  • BLS and/or ACLS as appropriate for the clinical role. Upon Hire Required and
  • Nurse Practitioner - board certified by American Nurses Credentialing Center Commission on Certification Upon Hire Required and
  • Nurse Practitioner - Completion of an accredited program for nurse practitioners Upon Hire Required or
  • Physician Assistant - Graduate of accredited physician assistant program Upon Hire Required
