Our Mobile Primary Care Team is seeking a dedicated and compassionate Physician Assistant or Nurse Practitioner with a heart for senior care and a passion for redefining how older adults and their caregivers experience health and well-being. Help solve the rural area access to care crisis in Colorado - this role is based in Pueblo, with travel to Trinidad, Rocky Ford, and Fowler to bring high-quality, relationship-based care to communities that need it most.

Your Responsibilities Include:- Delivering personalized high-quality care to seniors in their homes, assisted living, hospice, and nursing facilities.
- Traveling across Pueblo, Trinidad, Rocky Ford, and Fowler to extend care access to rural and underserved communities.
- Delivering dementia-informed, relationship-based care across the continuum of care.
- Conducting comprehensive assessments and developing tailored care plans for older adults.
- Managing chronic conditions and performing care with skill and empathy for patients and their families.
- Collaborating with cross-functional partners - including palliative, psych, and neuro - to deliver coordinated, whole-person care.
- Building trusted relationships with residents, caregivers, and facility partners to enhance engagement and referrals.
- Maintaining consistency in documentation, follow-through, and patient communication.

What You'll Bring- Active, unencumbered NP or PA license in the state you will be practicing.
- Minimum of 1-2 years of clinical experience (geriatrics or primary care preferred).
- Compassionate, patient-centered approach and strong clinical judgment.
- Valid driver's license and reliable transportation, with willingness to travel to Pueblo, Trinidad, Rocky Ford, and Fowler.
