The Advanced Practice Provider(APP ) plays a pivotal role in coordinating the continuum of care and managing the overall patient experience within the USC Brain Health Network (UBHN ), ensuring good communication, efficient and patient-centered operations, and effective service delivery for every patient. They are responsible for coordinating and navigating the care for all patients referred to the UBHN for services between the primary care provider and referrals to ancillary services and specialties. To demonstrate commitment to conducting business lawfully and ethically by consistently adhering to compliance policies, procedures and regulations.This position will travel across the state of South Carolina.
Minimum and Additional Requirements
Knowledge/Skills/Abilities
• Ability to communicate effectively.
• Ability to assess, identify, and address individual needs.
• Knowledge of or the ability to learn medical terminology, use medical equipment and follow clinical orders.
Preferred Qualifications

• Prefer at least one year of experience working in a physician practice.
• Prefer experience coordinating care of patients across different areas/services of a health system.
