Position Summary

Provides high quality patient care in their respective specialty to patients. C ollaborates and works effectively with other providers and staff to provide effective team based medical care.

Primary Position Responsibilities

Qualifications

Minimum Education:

Required:

Medical Degree: MD or DO OR Graduate Degree to fulfill current regulatory requirements to function as a mid-level provider.

Minimum Years of Experience (Amount, Type and Variation):

Required:

Residency and or fellowship training completion required by specialty or appropriate Mid-Level training.

License, Registry or Certification:

Required:

New Jersey Medical License; Board Certification or Board Eligibility in requisite specialty or appropriate mid-level New Jersey state license equivalent. New Jersey Controlled Dangerous Substance (CDS) License and Drug Enforcement Agency (DEA) License
