Full Job Description
As the Admissions Nurse, you will serve as both a clinical leader and community ambassador. You will be instrumental in helping residents and families navigate challenging healthcare transitions while collaborating with hospitals, physicians, case managers, and community providers to ensure a smooth and successful admission process.

What you'll do:
  • Review, evaluate, and process admission referrals in a timely manner.
  • Conduct comprehensive nursing assessments for prospective and high-risk admissions, including medication reconciliation.
  • Ensure appropriate clinical placement and coordinate bed assignments based on resident needs and acuity.
  • Collaborate with physicians, case managers, discharge planners, social workers, and healthcare providers to facilitate smooth transitions into the facility.
  • Assist with admission orders, nursing assessments, care plans, and other clinical requirements.
  • Verify and confirm all physician orders prior to admission.

Who We're Looking For:
  • Current Registered Nurse (RN) license in the State of Hawaii and in good standing.
  • Minimum two (2) years of Long-Term Care (LTC) nursing experience.
  • Experience in Community-Based Care, Care Coordination, Transitional Care, Case Management, or related healthcare settings.
