OverviewAt FutureCare the Admissions In-Take Nurse is responsible for timely pre-admission clinical review of potential patients, to include admission and re-admission screening tools, MARs and TARs, clinical and rehab notes, labs, and other necessary information.

Responsibilities
- Conducts timely review of pre-admission screening of potential patients, to include but not limited to, admission and re-admission screening tools, MARs and TARs, clinical and rehab notes, labs, and other necessary information.
- Along with Admission Coordinator, manage electronic referral systems (NaviHealth, Aiden, Allscripts, ETC) to ensure timely responses to referrals, and maintain effective communication with referral sources. Monitor and analyze data from reports.
- Communicate effectively with DON, Regional Care Managers, Admissions, Executive Director, Respiratory Director, and other team members, as well as hospital staff, patients and families.
- Provide oversite and training for the Admission Coordinator
- May contact payer source to determine or verify coverage and adequate funding for required patient services such as high cost drugs, DME or other necessary services and length of stay prior to admission
- May attend regional meetings, scheduled in-service programs, and other necessary meetings as directed by Executive Director and VP Business Development.
- Thinks and acts calmly and logically to meet unusual occurrences of the job without being thrown off stride
- Performs any miscellaneous work assignments as may be required
Qualifications
- Must have a current RN or LPN license in the state of Maryland
- Minimum one year experience in Admissions, long-term care and/or sub acute care
- Knowledge of ventilator and dialysis care preferred
- Strong oral and written communication skills, required
- Ability to sit, stand, and walk most of the time; may also require driving (Minimal travel as assigned by Director of Nursing
