Job Summary and Requirements

This opportunity

This posting will be used to fill ONE (1) Term position (anticipatory perm) until December 31, 2027, working 75 hours bi-weekly. It will establish a 12-month eligibility list which may be used to fill similar positions.

The job is based in the thriving city of Whitehorse in the beautiful Yukon Territory.

Who we are looking for

This position reports to the Supervisor, Home Care Interdisciplinary Team. The role is central to the client admission process for Continuing Care services, and involves reviewing and prioritizing referrals, completing comprehensive client assessments, and identifying complex care needs and risks to assist with determining appropriate levels of care for clients. Responsibilities also include facilitating client admissions and collaborating with a variety of internal and external stakeholders to support successful transitions in care. The position may also provide interim care coordination for clients not admitted to the home care program who are awaiting admission to programs such as respite care or permanent long-term care.

Essential Qualifications

  • Post secondary degree as a Social Worker (SW), Registered Nurse (RN), Occupational Therapist (OT) or Physiotherapist (PT)
  • Registration with a Canadian discipline-specific regulatory authority (see Conditions of Employment below)
  • Experience working in a health care environment
  • Experience working in a home care and/or long-term care setting is an asset
  • Experience doing standardized RAI-HC or InterRAI-HC assessments is an asset
  • Familiarity with the Care Consent Act and provision of "Capability to Consent to Care in a Care Facility" assessments is an asset

Conditions of Employment

You must have the following before being hired into this position or within the time specified:
  • Current registration with a health professional regulatory body
  • Vulnerable Sector RCMP security clearance
  • TB screening
  • CPR level C with AED (completed within last 30 months)

  • Valid class 5 driver's licence
