Job Description

We are seeking a passionate and dedicated Acute Care Physical Therapist to join our team. The ideal candidate will have a strong passion for patient care and a thirst for continued knowledge and growth.
  • Assess patients to develop and initiate individualized treatment plans based on evaluation findings, patient needs, and goals
  • Provide direct patient care using established protocols, evidence-based interventions, and patient care guidelines
  • Document evaluation findings, treatment progress, and instructions provided to patients and caregivers accurately and timely
  • Provide appropriate oversight of patient care performed by physical therapist assistants and rehabilitation technicians
  • Complete a written plan of care, establish measurable goals based on evaluation findings, and develop a comprehensive treatment program to achieve those goals
  • Implement individualized physical therapy treatment programs to improve mobility, function, strength, and overall independence
  • Communicate effectively with patients, caregivers, and professional colleagues, including physicians, nursing staff, case management, and insurance providers, to coordinate care and support optimal patient outcomes
  • Complete required documentation, including progress notes, treatment summaries, and discharge recommendations, to support continuity of care
