Fall River Nephrology | Panoramic Health

Location: Fall River, MA

Compensation: $120,000 - $130,000 annually, depending on experience, plus comprehensive benefits including annual bonus opportunities.

Fall River Nephrology, a member of the Panoramic Health network, is seeking a compassionate and motivated Nurse Practitioner to join our growing nephrology team. This is an excellent opportunity to work alongside experienced nephrologists while caring for patients across outpatient clinics, dialysis centers, and hospital settings.

Position Summary

The Nurse Practitioner will provide comprehensive care to patients with chronic kidney disease (CKD), end-stage renal disease (ESRD), hypertension, and related conditions. The provider will collaborate closely with nephrologists, dialysis teams, hospital staff, and community providers to deliver coordinated, patient-centered care.

Key Responsibilities

Outpatient Nephrology Clinic
  • Evaluate, diagnose, and manage patients with chronic kidney disease and related disorders
  • Develop individualized treatment plans focused on preserving kidney function and improving outcomes
  • Monitor laboratory studies, diagnostic testing, and medication therapies
  • Educate patients and families regarding disease management and treatment options
  • Coordinate referrals, follow-up care, and specialty consultations as needed

Dialysis Care
  • Manage hemodialysis and peritoneal dialysis patients in accordance with established clinical guidelines
  • Assess and treat complications related to ESRD, including anemia, hypertension, mineral bone disease, electrolyte disorders, and vascular access concerns
  • Conduct routine dialysis rounding and monitor patient progress
  • Collaborate with interdisciplinary care teams to optimize treatment plans
  • Support patients and families through treatment decisions and ongoing education

Hospital Responsibilities
  • Perform patient evaluations, histories, and physical examinations
  • Participate in inpatient nephrology management and care coordination
  • Assist with admissions, discharge planning, and transitions of care
  • Collaborate with physicians, nurses, case managers, and other healthcare professionals to ensure continuity of care

Qualifications

Required
  • Master's or Doctoral degree from an accredited Nurse Practitioner program
  • Current Massachusetts Nurse Practitioner license or eligibility for licensure
  • Adult-Gerontology Acute Care NP Certification (AGACNP)
  • Current DEA registration or ability to obtain
  • Strong clinical assessment, diagnostic, and patient management skills
  • Excellent communication and collaboration abilities

Preferred
  • Experience in nephrology, dialysis, internal medicine, hospital medicine, primary care, or chronic disease management
  • Familiarity with electronic medical records
  • Passion for caring for patients with complex chronic conditions

This role has a salary range of $120,000 and regular, full-time employees working 30 or more hours per week are eligible for comprehensive benefits including Medical, Dental, Vision, Life, 401(K), Paid time off (PTO).
