A bit about this role:
This position represents an amazing opportunity for an ambitious APP leader ready to take on a crucial role building out and leading the Active Care Service for Devoted Medical Primary Care.
As an Active Care APP Manager, you will be responsible for the clinical and operational oversight of a team of Advance Care Providers (APP) who will deliver world class primary care to Devoted Health Plan members. This position also includes direct clinical responsibilities in caring for our patients.
On a day-to-day basis you will work closely with the primary care leadership team, the APPs on your team, and cross functionally to deliver on our ambitious objectives. This is an amazing opportunity for professional growth within the organization.
You will be building a best in the nation virtual primary care practice supported by advanced AI-enabled technology, a network of clinical and community outreach professionals, and comprehensive Devoted Medical programs 6 including virtual urgent care, specialty care such as behavioral health, complex care management, and clinical pharmacy services. This position offers the opportunity to impact patient outcomes at scale in a modern, tech-supported care environment.
You will report directly to the Medical Director for primary care.
Overview:
This role will be APP Manager on the Active Care service. Active Care is one of the core teams within the primary care departments. The Active Care team is responsible for stabilizing and managing complex primary care patients with multiple chronic conditions. Active Care works with a panel of members with uncontrolled chronic disease, acute disease in need of investigation and treatment, and preventative health needs in a longitudinal care model alongside other APPs and Physicians. This role will launch in the Summer/Fall of 2026 and this role will serve as a crucial contributor to the buildout of the delivery model.
This role is for a clinical leader who develops, supports and guides a team of APPs providing primary care through telehealth while also leading process improvement projects that help develop our primary care operation. The APP manager will excel at change management, communication, and culture building in a rapid change environment. The APP Manager will also consistently ensure their team delivers high-quality, member-centered care, adheres to workflows, meets program expectations and continues to grow clinically.
This role requires strong communication skills, process improvement skills and a focus on performance coaching. You will also maintain a limited panel of visits to stay connected to clinical work and program workflows.
Your Responsibilities and Impact will include:
Team Coaching, Development & Support
Provide ongoing coaching to support strong clinical reasoning, member communication, documentation quality and overall member-centered care.
Conduct regular 1:1s to ensure providers feel supported, clear on expectations and connected to the team.
Offer clear and timely feedback when performance, behavior or workflow adherence needs improvement.
Develop and execute individualized coaching and strategic action plans to help APPs meet performance expectations and achieve growth goals.
Conduct annual performance reviews to assess progress, celebrate strengths and identify areas for development.
Foster a team environment rooted in transparency, shared learning and psychological safety.
Performance & Quality Oversight
Monitor performance trends and visit quality using dashboards, documentation review and direct observation.
Identify performance variation early; reinforce strengths and address gaps with collaborative improvement planning.
Ensure providers maintain consistent workflow adherence, productivity expectations, time management and quality of member interactions.
Assist in developing and refining standard operating procedures (SOPs), ensuring that our workflows stay aligned with our mission as our program grows and evolves (we build the plane while we fly it).
Partner with leadership and clinical programs to support ongoing upskilling and shared best practices.
Program Design
Collaborate closely with primary care leadership to define the key features of the Active Care Service
Vision and design new workflows to support our core objectives. Lead the implementation of those new workflows and ensure they are effective by closely monitoring effectiveness
Foster a culture of interdisciplinary cross-team collaboration between the Active Care team and the seven other teams closely supporting primary care patients.
Operational & Scheduling Support
Monitor team capacity, schedule coverage, visit volume patterns and PTO to ensure stable access to care.
Collaborate with scheduling and operations partners to balance provider availability and program needs.
Support onboarding and orientation of new clinicians, helping them build confidence with systems and workflows.
Assist with onboarding ramp-up plans, ensuring new hires are integrated successfully and supported through their early months.
Participate in interviewing and selection of APP candidates, helping ensure strong clinical and cultural fit.
Engage in special operational projects and process improvement initiatives that strengthen team efficiency and member experience.
Clinical Responsibilities
Complete a smaller number of weekly clinical visits flexing based on staffing and program needs.
Coordinate care with interdisciplinary teammates, external providers and caregivers.
Utilize Devoted9s internal clinical systems and contribute feedback that improves tools and workflows over time.
Required skills and experience:
Prior primary care clinical background with strong management experience
3+ years NP clinical experience in relevant care settings.
Demonstrated experience leading change
Strong track record supporting, mentoring or developing clinicians.
Stellar communication skills and comfort navigating change.
Experience working in interdisciplinary teams.
Fluid usage of the Google suite of products.
Active, unrestricted RN + APRN licensure and national certification.
Willing to obtain additional state licensure with support.

Salary range: $121,000 - $171,720 annually
The pay range listed for this position is the range the organization reasonably and in good faith expects to pay for this position at the time of the posting. Once the interview process begins, your talent partner will provide additional information on the compensation for the role, along with additional information on our total rewards package. The actual base salary offered will depend on a variety of factors, including the qualifications of the individual applicant for the position, years of relevant experience, specific and unique skills, level of education attained, certifications or other professional licenses held, and the location in which the applicant lives and/or from which they will be performing the job.
