Job Title: Account Manager Company: Health Link Home Health and Hospice Location: South Bay Area, CA Pay: $40-$45/hour (based on experience) Job Type: Full-Time

The Role

As an Account Manager, you will be the face of Health Link in the field - building lasting relationships with referral sources, healthcare providers, and community partners. Your work directly connects patients to the care they need. You will partner closely with the Director of Community Relations to develop your territory, grow our referral network, and position Health Link as the first choice for in-home care across the South Bay.

What You'll Do
  • Develop and execute a sales and outreach strategy to grow Health Link's referral base across your assigned territory
  • Build and maintain relationships with hospitals, clinics, physician offices, skilled nursing facilities, senior communities, and other healthcare organizations
  • Represent Health Link at community events, networking functions, and industry meetings
  • Conduct outreach calls, schedule in-person meetings, deliver presentations, and follow up consistently to turn leads into lasting partnerships
  • Coordinate with the intake team to ensure a smooth referral process and serve as the communication bridge between partners and internal staff
  • Track all outreach activities, contacts, and outcomes using CRM software
  • Manage a personal outreach budget responsibly
  • Participate in ride-alongs and training sessions with senior team members
  • Stay informed on industry trends and competitor activity
