SALARY RANGE: $80,000-$85,000

POSITION SUMMARY:

The CMA-PSO is looking for a remote California-based Practice Transformation Specialist to help facilitate transformative change within private practices and community health centers (CHCs) on a few large-scale projects. As a Specialist, you will conduct frequent virtual or (permitting) in-person visits to your assigned practices and health centers, assisting the physicians and their staff with transformation plans to shift into value-based care, improving quality and encounter data submissions, identifying opportunities for increased practice efficiency and leveraging health IT and data to deliver high-quality, culturally competent, equitable, and comprehensive primary health care, with a specific focus on improvements in clinical quality, patient-centered care, and provider and staff well-being. In addition, the Specialist will help CMA-PSO develop its practice transformation process and tools, aimed at building practices’ overall capacity for ongoing and sustainable change to high-performing medical practices.

DUTIES/RESPONSIBILITIES

The Specialist will be expected to manage their assigned practices and health centers on multiple projects, providing technical assistance including workflow changes, conducting assessments, supporting interventions with different practice and/or health center staff, improving the utilization of digital health tools, reviewing clinical policies and protocols and workgroup participation when necessary. The Specialist will drive the practices and health centers toward outcomes that improve care and reduce costs. As a Specialist, you will partner with your assigned practices and health centers to help design, deploy, and train staff on workflows to incorporate various technologies and processes into their daily activities. The mission is to change the landscape of healthcare delivery for that practice or health center.

KNOWLEDGE/EXPERIENCE/EDUCATION

At least two (2) years of prior experience working directly with small-, medium- or large-sized Federally Qualified Health Centers/Community Health Centers (FQHCs/CHCs) across California, preferably helping them with quality improvement (QI) and/or practice transformation initiatives.
One to three (1-3) years of prior experience working directly with independent practices supporting them on QI and/or practice transformation initiatives.
Solid understanding of the healthcare delivery system, particularly medical management, value-based care, Medi-Cal patient populations, population health management (PHM) and health information exchanges (HIEs).
Based in California (required) and willing to travel occasionally for practice and health center onsite meetings (Note: It is expected that most of the work will be remote, requiring virtual visits).
Education: College degree, preferably in a healthcare field of focus
Strong project management experience working on medium to large scale initiatives, preferably in a practice/clinical setting. Includes experience conducting the following:
Facilitating assigned practice and health center meetings, including meeting agendas, minutes, handouts, and monitoring action items to completion.
Coaching practices and/or health centers on how to improve quality and progression toward meeting value-based care goals.
Providing updates, reports and/or executive level summaries to medical groups and/or health plans that are stakeholders in the initiative(s).
Experience exporting data and running reports, preparing proposals, reports, updates, and summaries in the practice/clinical setting, process mapping, and workflow analysis tools.
Understanding of and previous experience working with Electronic Medical Records (EMR), Practice Management Systems (PMS) and Population Health Management (PHM) tools for clinical purposes.

Highly organized.
Great bedside/working manner and strong relationship building skills.
Passionate about population health and helping practices and/or health centers improve patient care.
