Responsible for the delivery and documentation of pharmaceutical care provided to patients using the medication use process (selecting, prescribing, dispensing, administration and monitoring of medications).
Collaborate with health plan and pharmacy director on training materials to provide plan overview of program, benefits, formulary and PA Guidelines to the specific state requirements for the Pharmacy Call Center Team
The Pharmacist will work as an integral member of the healthcare team at a large NCI designated Cancer Center to optimize patient outcomes through drug therapy monitoring, guideline development/implementation, and patient education.
Performs prospective and/or retrospective drug utilization review and appeals per State and Federal regulations; participates in the distribution of clinical drug information and educates providers and staff regarding the use of pharmaceutical treatment regimens.
The Physician is responsible for the medical services to children in an on-site children's residential facility. Provides for services during an assessment, intake and daily medical needs of the children during their stay at the facility.
The physician provides comprehensive evidence based care to a designated patient population, providing gastroenterology services to patients in the medical office setting as well as to hospital patients on an inpatient and outpatient basis.
