Responsible for overseeing the development, operations and advancement of Geisinger’s system wide Geisinger at Home (GaH) care model for the highest risk, most complex population. Administrative accountability for operations including risk stratification, population identification, enrollment and patient/family engagement and a 24/7 centralized call center that supports advanced clinical triage to patients and their families. Provides day-to-day direction for clinical informatics identifying, monitoring and assuring data transparency for the GaH team. Has marketing and communication responsibilities for GaH including community-based outreach to physicians and other community partners.
Works with executive leadership within GaH to assure alignment across the business needs for the Clinical Enterprise, Health Plan and the region’s Keystone ACO (KACO) while serving the highest risk patient population. This executive is responsible for the development of the overall strategy, integration and execution of operational goals for Geisinger at Home and subsequent integration with Geisinger’s Care Care Coordination and Integration teams as well as Geisinger’s Placement Center.
Ensures the program’s goals and objectives are aligned with the organization's broader mission and vision. Partners with GHP, hospital based Medical Directors and Geisinger Clinic Medical Directors to analyze value and impact medical trajectory trends and support the Triple Aim – better outcomes, optimal experience and affordable care. Works with Geisinger and GHP leadership to plan and drive new innovative care solutions that are value-based, cost effective and lead in their market. Along with the senior leadership team of GaH, has P& L accountability for cost management and medical cost reduction for GHP and KACO
Accountable to the Chief, Geisinger at Home with a matrixed relationship to the CMO.
MAJOR DUTIES AND RESPONSIBILITIES
1.Responsible for operational activities and functions within GaH including the intake/enrollment team, triage team, and clinical informatics as well as budget accountability for GaH.
2.Works to create a comprehensive marketing plan to fully inform the provider community as well as other agencies of the program capabilities. Develops and maintains program promotional materials related to program description, enrollment and services.
3.Responsible for strategic and tactical planning, including, but not limited to patient/family engagement and navigation services, further product development and implementation, vendor relations and internal and external provider relations.
4.Supports regulatory requirements related to GaH.
5.Leverages health information technology and workflows to ensure value-based outcomes, and effective, efficient and cost-effective use of resources in order to maintain a productive environment.
6.In conjunction with clinical leadership identifies key performance indicators. Accountability for the development and monitoring of KPIs to aid in monitoring and achieving desired performance outcomes.
7.Provides direction to, holds accountable, develops and evaluates staff in their performance of all goals and objectives of the service line and in conjunction with the medical directors
8.Evaluates regularly the effectiveness of GaH and implements changes as necessary to promote strategic and operational effectiveness.
9.Establishes a matrix working relationship between GaH and other stakeholders across Geisinger.
10.Creates an integrated leadership team among physicians and non-physician staff to ensure achievement of organizational goals and objectives. This includes a leadership development plan for the next level of leadership and a succession plan.
11.Performs other duties as assigned.
SKILLS AND ABILITIES:
Demonstrated leadership, facilitation, and networking skills. Must have ability to think strategically and possess leadership effectiveness competencies.
Must have ability to work in a matrix relationship and demonstrate ability to lead and manage significant change activities.
Demonstrated analytical and financial management skills (operating and capital budgets).
Demonstrated skills in the development and optimization of health information technology solutions
Demonstrated competence in written, oral and electronic communication skills (interpersonal and technological effectiveness competencies).
EDUCATION AND/OR EXPERIENCE
Master’s Degree in Health Care/Health Care Administration or Business required or near completion.
Minimum 5 - 8years’ experience in a senior leadership role, preferably as part of a large, multi-system medical center or integrated health system. Experience with population health, medical cost management recommended. Managed care experiencepreferred.
WORKING CONDITIONS/PHYSICAL DEMANDS:
Work is typically performed in an office environment. Independent travel to the sites throughout the system and surrounding communities required.
