BASIC FUNCTION:

This position is responsible for implementing, coordinating, and maintaining Member Care Coordination strategies and programs to support government programs and enhancing health care outcomes for members; achieving cost containment goals and objectives for the department; managing teams involved in member care coordination and representing the company and division before outside customers and consultants.

JOB REQUIREMENTS:

*Registered Nurse (RN) or Licensed Social Worker currently licensed and in good standing n the state of operations.
*3 years of clinical or medical/surgical experience.
*2 years case management program oversight experience.
*1 year supervisory or management experience.
*Knowledge of managed care programs.
*Knowledge or experience using accrediting agency standards.
*Program development or project management leadership experience.
*Written and verbal communications skills; interpersonal and organizational skills.
*PC skills and experience to include Microsoft suite of applications.
*Ability and willingness to travel, including overnight, as required.

PREFERRED JOB REQUIREMENTS:

*Case Management Certification (CCM).
*LISW
