Collaborates and coordinates with multi-disciplinary teams to facilitate a safe quality experience for patients though the continuum of care. BSN is required. (Internal applicants currently in a Care Manager position who are enrolled in a BSN program may apply). Knowledge of MS Office; a minimum of 3 years clinical experience is required. Two years in a Leadership Role is preferred. Experience in Care Management is also preferred.
