The Transitional Care (TC) Director will oversee numerous special projects focused on driving the success of our Transitional Care business. Additionally, will support TC leadership (GM & CMO) in executing on strategy, vision and collaborating closely with the field leaders to launch, oversee and measure the success of pilots and programs nationally.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Support strategic planning and operational execution of programs, pilots or projects
Engage applicable stakeholders to create buy-in, and understand various stakeholder needs and cost/benefits of specific approaches
Communicate proposals and recommendations for desired plans to key executives
Develop, communicate and implement strategic projects; articulating problems, potential solutions, risks, management plan and timelines.
Oversee development of support materials, execution plans, and all other documents and tools that are needed to manage the TC program
Gather and disseminate BDPs across field regional teams; support physician engagement strategies and execution
Build relationships with and manage partners and vendors
Support field teams (operators and clinicians) with region-specific operational issues and requests and local needs
Support business development and advocacy efforts
Provide performance monitoring, information and reporting; help diagnose root cause of problems.
Support cross-functional teams across data/analytics, IT, advocacy, finance and business development to ensure teams are effectively delivering on key Transitional Care goals and programs; Relieve bottlenecks and complexities

Ability to work effectively with Sound's business colleagues, as well as with various types of clinicians, including physicians, nurses, and nurse practitioners in the field

EDUCATION / EXPERIENCE:
Minimum: Bachelor's Degree in business administration, healthcare administration or similar field
Preferred: MBA, MHA
Minimum: 2-3 years of consulting, operations and/or broad scale implementation experience
Preferred: 3-5 years' of consulting, operations and/or broad scale implementation experience
SPECIAL JOB OR ENVIRONMENTAL CONDITIONS:
Flight Travel: Willingness to travel up to 50%
