Qualifications

• Active RN license in the state of California
• 5+ years of nursing experience, preferably in case management
• Strong understanding of patient advocacy and treatment protocols
• Excellent communication skills for interacting with patients and families
• Ability to work independently and in a team environment
• Experience with electronic health records and care coordination

Responsibilities

• Act as a patient advocate throughout the healthcare process
• Educate patients and families on health statuses and treatment options
• Coordinate necessary resources and services for patients
• Assist patients in following through with their treatment plans
• Develop and maintain patient care plans based on individual needs
