Job Description
TheRNCare Manager isresponsible for the provision of care management/utilization review as well asadmission and discharge planning, ensuring effective and efficient patient progression through the continuum in this special pediatric healthcare environment.   A key function of this role is to engage in proactive consultation  with other members of the multi-disciplinary team, including but not limited topatient care staff, physicians, family; and, community resources to assure a smooth transition for the patient through appropriate levels of care in order to optimize quality outcomes.  TheRNCare Manager may berequiredto participate in outpatient clinics, outreach clinics and school or community re-entry programs.



Job Skills & Knowledge
Must be able to demonstrate effective verbal and written communication skills with a wide variety of staff, patients, volunteers and medical staff.  Must demonstrate the ability to manage competing priorities in a professional effective manner.  Must be able to work  effectively in both  a multi-disciplinary team based environment or independently ofsupervision.  Must be able to recognize and facilitate constructive change within an environment of safe, qualitypatient care.  


Job Requirements


Minimum of 5 years of experience as a Registered Nurse  
Graduate from a BSN or MSN program

RN Licensure in good standing with the State of Texas 
Certification as a Case Manager within 3 years of employment 
Completion of American Heart Association Basic Life Support for Healthcare Provider Program required within 30 days of employment

Proficiency in Microsoft Office, specifically Excel


 Experience as a care manager highly desirable

 Bilingual in English/Spanish
 Strong working knowledge of revenue cycle processes and applications preferred
