Description of the role:
University Home Care is seeking several Physical Therapists to cover the Lincoln Park, MI and surrounding cities. As a Physical Therapist, you will be responsible for providing high-quality patient care, promoting optimal health outcomes, and assisting patients in improving their physical well-being.
Responsibilities:
- Assess and evaluate patients' physical conditions and develop personalized treatment plans
- Implement evidence-based physical therapy interventions to improve patients' mobility, strength, and overall physical function
- Monitor and document patients' progress, adjusting treatment plans as necessary
- Educate patients and their families on proper techniques for exercises and activities
- Collaborate with other healthcare professionals to ensure coordinated patient care
- Maintain accurate and up-to-date patient records
- Valid license to practice as a Physical Therapist in the state of Michigan
- Minimum of 2 years of experience in a similar role (home care experience required)
- Excellent knowledge of physical therapy principles, techniques, and practices
- Strong interpersonal and communication skills
- Ability to work independently and as part of a team
