Manage daily operations of pharmacy department. Participates on committees and assures patient safety through review of processes in assigned areas as evidenced by lack of errors or events. Facilitates the development and implementation of treatment guidelines, protocols, formulary changes and critical pathways as approved by appropriate committees as evidenced by meeting minutes. Provides clinical consultation and clarification to practitioners and patients. Suggests appropriate, cost effective therapeutic alternatives to medical staff as needed as evidenced by intervention reports. Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, adverse drug reactions and appropriateness of drug and dose as evidenced by eventreports. Facilitates the identification, resolution, implementation, and communication of cost-effective medication use issues in collaboration with the medical staff. Supports the pharmacy's pharmacoeconomic efforts and targeted compliance programs as evidenced by intervention reports. Other duties as assigned
5years Health Systems/Hospital Experience to include 3years Supervisory Experience
BS in Pharmacy from and ACPE Accredited Institution
ASHP Accredited Residency highly preferred
Virginia Board of Pharmacy Licensure or eligibility to obtain within 30 days of hire.
