. Actively supports and advances the Quality Work Plan for improving department performance and medication safety
Works with Director and others to plan, implement, analyze, monitor and maintain a safe and effective medication use process.
Obtains and analyzes data to assess department performance as needed or requested.
Develops and maintains competency with hospital computer system to enable identification of safety issues and assist other health care professionals in safe use of the system.
Uses Safety Tracker to report interventions and medication occurrences. Uses other documentation systems as requested.
Helps identify critical areas needing improvement.
2. Provision of clinical and educational services to patients and other healthcare professionals
Provides clinical and educational services such as rounding, drug information, patient education and optimization of drug therapy to patients and other health care professionals in selected practice site.
Serves as consultant to pharmacists and department in areas of clinical service. Assists pharmacists in resolving problems related to physician orders, therapy, medication preparation and P&T Committee requirements.
Acts as a liaison between Pharmacy and medical and nursing staff on issues pertaining to clinical services or other pharmacy issues.
Educates pharmacists and other healthcare providers in areas of pediatric drug therapy through inservices, presentations, written educational ,materials and other methods. Two CE programs a year for pharmacists are required.
Participates in training of pharmacy students and residents. Acts as preceptor as needed or requested.
Participates in hospital committees, working groups, councils and teams as needed or requested.
3. Works to promote effective medication use
Assists with provision of background material for Pharmacy and Therapeutics (P&T) and other committees, councils, working groups and teams as needed or requested.
Actively supports changes in medication use patterns promoted by Pharmacy, P&T, Medication Safety to increase patient safety, improve patient care, comply with law and regulations or meet other clinical and fiscal goals.
4. Assists with coordination of department services to provide optimal patient care with available resources.
Works with managers to assure safe and efficient patient care. Works to minimize disturbances in work flow and unplanned events.
Performs assigned duties to meet routine and emergency situations. Covers essential services like transplant notes for other coordinators in other practice areas when requested.
Substitutes as staff pharmacist and assists with distributive workload as requested by managers.
Assists in analysis, planning and implementation of new or revised services.
5. Continues professional development and exhibits leadership
Maintains and improves expertise in pediatric pharmacotherapy and in specialized area of practice. Meets or exceeds department and regulatory standards for clinical education.
Serves as role model and mentor for staff, residents, and students by consistently exhibiting professionalism. Precepts students and residents as assigned. Trains staff as requested.
Performs other duties as assigned.
Required - Bachelor's degree from four-year college or university - Field of Study: Pharmacy (B.S. or Pharm.D in Pharmacy required)
Preferred - Master's degree or equivalent degree - Field of Study: Pharmacy (Post graduate degree and/or residency or fellowship preferred)
Required - 3 years total experience required, which includes 3 years of experience in the area of specialty or 1 year post-graduate advanced training (ex: PGY1, fellowship, or similar) experience
Preferred - Additional pediatric hospital experience preferred
CERTIFICATION / LICENSE / REGISTRY REQUIREMENTS
Required - Pharmacist License: (AR) (Candidate must possesss at time of hire)
Preferred - BCPS - Pharmacotherapy Specialist (Certification)
Required - Excellant communication and people skills
Required - Attention to detail, ability to supervise and precept, work well with others
Required - Decision making and organizational skills
