Peds Heme/Onc Pharmacy Clinical Specialist
The Pharmacy Clinical Specialist will collaborate with physicians, nurses, and other allied health providers to provide excellent pharmaceutical care and services in the Pediatric Hematology/Oncology specialization. The Clinical Specialist is responsible for ensuring safe, appropriate, cost-effective medication therapies for patients according to established policies, procedures, and protocols; providing accurate, adequate, and timely drug information; providing staff education to physicians, nurses, and pharmacists; responsible for supporting quality initiatives through data analysis and management; and supporting effective operation through policy development, staff engagement, and expeditious medication order verification and delivery.
Provides oversight and assists with the development and implementation of evidence-based medication guidelines/protocols.Supports quality initiatives through data analysis and management. Develops criteria for appropriate use of medications.Participates in Pharmacy and Therapeutic Committee (P&T) activities, as appropriate. Evaluates medications for formulary additions.Functions as a therapeutic consultant in areas that may include rational and appropriate use of medications, clinical pharmacology, pharmacokinetics, disease state management, and nutrition. Provides clinical interventions and counseling.
3 years of In-Patient Pharmacy experience to include in a Pediatric Hematology/Oncology setting
1 year of experience as a clinical pharmacy specialist
Completion of an ASHP-accredited PGY-1 Pharmacy Residency with at least 3 years of practice in a Peds Hematology/Oncology or directly related setting OR completion of a PGY-2 Critical Care Pharmacy Residency (with a focus in Pediatric Hematology/Oncology).
Graduation from and ACPE Accredited Institution;
BLS and ACLS certifications
